How are Libsyn upload errors fixed?

Errors when uploading to Libsyn can usually be solved by one of the following:
You need to Reconnect Libsyn connection (this would usually resolve the problem and if it still persists, proceed to step 2)
After Reconnecting Libsyn, you can try to republish an episode to see if it goes through.
Monthly quote is exceeded in Libsyn. You need to delete files from Libsyn or upgrade your account.
